I am sorry but maybe I misinterpreted your question. I did not mean it
is possible by using the ffmpeg command line tool but theoretically
doable. That's how I interpreted the question "Is doing something like
that even possible?".

If you want to do it yourself, a fair bit of coding is required and
you would have to read the mp4 file specification to understand how
that will work.
