Thanks for your response Carl.
Yes I am using "-async 1" for A/V sync.
Attached is the sample where I see the issue.
It has some audio at 11s, 22s and 40s.

Please try current git head and / or provide the input sample.
(Why do you use -async 1? Is it needed for A/V sync?)
