Michael fixed -ss and codec copy recently (a/v desync). -ss + codec copy
won't be accurate (cut at key frames) but very fast, -ss + transcode will
be accurate because it can cut at any frame. You might want to mention
this in the next patch. Note that I don't know how different it performs
with -ss as input and output in these cases.
