I'd take the sane-nonblocking-environment's way:

Reviewer gave his OK -> sane timeout (a week?)/commit menace by author
-> author commits

plus:

Reviewer fails to respond to review request -> sane timeout (a
month?)/commit menace by author -> author commits if no objections
had been raised before by any other developer.

Timeouts can obviously be adjusted to meet a commonly agreed criterion.
