Seems you're not done yet, Michael ;)

See incoming/I_PCM_decoding_error_2.h264 on mplayerhq FTP.  This
decodes incorrectly in both Elecard Streameye and latest libavcodec,
but JM says the stream is fine.  Given the problem and corresponding
solution previously, I can't guarantee this is a problem with PCM
macroblocks either, but I'll put it in the same thread since it seems
somewhat on-topic.

Of course this time, its CABAC, not CAVLC.
