This is unnecessarily convoluted.  Try to keep your sentences as simple
as possible.

  In addition to the first video and audio streams, the resulting
  output file @file{test12.avi} will contain the second video
  and the second audio stream found in the input streams list.

Do not put a comma before and in enumerations.

No need to send further patches, just commit with these changes.
