It indeed mentions the ctts/b-frames issue (which I wasn't even aware
of) and a "VOL" issue. I was merely talking of h264 in mp4, which seems
to need an "avcC" atom that ffmpeg does not insert.

Anyway, the book is underway (hope it will be useful), I will try to
address all these issues.
