[FFmpeg-cvslog] fftools/ffmpeg: rework handling -max_error_rate

fftools/ffmpeg: rework handling -max_error_rate
Replace the decode_error_stat global with a per-input-stream variable.
Also, print an error message when the error rate is exceeded.
