[FFmpeg-cvslog] avformat/utils: Move ff_add_attached_pic to demux_utils.c

avformat/utils: Move ff_add_attached_pic to demux_utils.c

It is demuxer-only: It potentially adds an AVStream and it sets
AVStream.attached_pic.
