[FFmpeg-cvslog] lavc: deprecate avcodec_get_frame_defaults().

lavc: deprecate avcodec_get_frame_defaults().

Also bump libavcodec micro and add an APIchanges entry saying that
av_frame_* should now be used instead of the lavc AVFrame functions.
